Case management practices
Out of the six studies, four were nurse-led. However, all of them were conducted with an interdisciplinary team comprising case managers, physicians, social workers, and nurses. The case managers were either pharmacist case managers, social workers, or nurse case managers, and they were the practitioners in charge of the primary tasks. The interventions were initiated either during the assessment of hospital discharge or immediately after, and the discharge and focus were always on the transition from hospital to community care (Chow & Wong, 2014; Farris et al., 2014; Meisinger et al., 2013; Melton et al., 2012; Reinius et al., 2013; Sandberg et al., 2014).
Duration of case management
The case management interventions were conducted over a duration ranging between six months and five years. The mean duration over which follow-up was conducted was 1.75 years (Chow & Wong, 2014; Farris et al., 2014; Meisinger et al., 2013; Melton et al., 2012; Reinius et al., 2013; Sandberg et al., 2014).
