I am sorry to hear that you have been facing some health issues, and I am glad to hear that you are doing better.

Regarding your on-ground class, we (the Dean’s office) understand your situation and want to be supportive, however, we are not in favor of the idea of transitioning your on-ground course to fully online. Could you teach your on-ground course in an online format for two weeks? However, if you believe that you are going to need more than two weeks to recover, then you may want to consider Family and Medical Leave (FMLA).
